Referrals accepted from family physicians only after neurologist diagnosis and the patient must have insight into / accept their diagnosis.
The clinic is for treatment, not diagnosis confirmation or second opinion
Patients must want treatment
Rehabilitation is often important
Community Rehabilitation Interdisciplinary Service - Central Access and Triage (CRIS - CAT) : before referring, review eligibility criteria carefully
Accepts FND patients on a case-by-case basis
Patients must need combined treatment of PT / OT/ SLP NOT just PT

Functional Neurological Disorders Clinic at South Health Campus - research and tertiary care program that assesses patients with FMD for suitability to rehabilitation programs. Referral required (see link to Alberta Referral Directory).
